Job summary

Waystone Health & Human Services in Haverhill, MA is seeking a Day Habilitation Program Supervisor to lead daily operations, supervise staff, and ensure safe, compliant services for individuals with ID/DD. This role requires coordinating program activities and maintaining open communication with families and guardians.

The position supports ISP/DHSP development, interdisciplinary team collaboration, and adherence to regulations while fostering growth and continuous professional development

Qualifications

• Bachelor’s degree in Human Services or related field with at least three years of ID/DD experience.
• High School Diploma or GED with at least five years of ID/DD experience.
• Bilingual English and Spanish preferred.
• Minimum of two years supervisory experience.
• Experience implementing and monitoring Individual Service Plans (ISPs) or similar plans.
• Familiarity with Positive Behavior Supports (DDS) and EHR platforms.

Responsibilities

• Hire, supervise, evaluate and train Case Managers and Developmental Specialists.
• Create and implement curriculums and activities that support the interests and needs of individuals served, ISP goals and compliance.
• Coordinate monthly Interdisciplinary Team Meetings to align clinical and behavioral services with identified needs.
• Ensure timely submission of program data for individuals receiving services.
• Support development and ongoing review of Day Habilitation Service Plans (DHSPs) and ISPs.
• Monitor and reinforce consistent implementation of approved strategies to promote positive outcomes and regulatory compliance.
