We provide medical care, in‑home support, hospice and palliative care, cancer navigation, and 200+ other free services to qualifying seniors. Every service is delivered at no cost, regardless of insurance status.

Role Overview:

As Data Analyst / Outcomes Specialist, you will measure and report on the impact of our programs across home health, hospice, community services, and grants. You will build dashboards, run quality analyses, and support grant reporting with rigorous outcome data.

Key Responsibilities:
  • Build and maintain dashboards tracking patient outcomes, program participation, and quality metrics
  • Analyze HHCAHPS, Star Ratings, and OASIS data to identify quality‑improvement opportunities
  • Produce monthly impact reports for leadership, the Board, and funders
  • Support grant reporting with outcome data, statistical analysis, and visualizations
  • Collaborate with clinical leaders on data definitions, collection processes, and reporting needs
  • Maintain data integrity and ensure HIPAA compliance across data systems
Qualifications:
  • Bachelor's in statistics, public health, health informatics, or related field
  • 2+ years data analysis experience; healthcare or nonprofit a plus
  • Proficiency with SQL, Excel/Google Sheets, and a data viz tool (Tableau, Power BI, Looker Studio)
