Job summary

Eliot Community Human Services, Inc. in Danvers, MA, is hiring a Care Manager - CSA to coordinate family-centered behavioral health services for youth up to age 21. You’ll conduct home-based assessments, develop ICPs, and navigate wraparound planning with families and team members.

The role requires a bachelor’s degree in human services, a valid driver’s license, and experience with child mental health and crisis management. Bilingual candidates may earn a salary increase; travel is required.

Qualifications

• Bachelor’s degree in human services or related field.
• Must have a valid driver’s license and reliable transportation.
• Experience navigating child/family service systems.
• Experience working with child mental health issues and crisis management.
• Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase.

Responsibilities

• Serve as key contact for the youth and family to the Eliot CSA and its services.
• Initiate care planning activities with all members of the family and/or caregiving system.
• Conduct a comprehensive home-based assessment in conjunction with other CSA Team members inclusive of the CANS, CRAFTT (for youth 12 and older), SNCD and other tools as determined necessary, which occurs in the youth’s home or another location of the family’s choice.
• Initiate planning that includes intake meetings, orient the family and the member to the wraparound process, identify strengths and needs of the child.
• Develop and implement a youth and family centered Individualized Care Plan (ICP) in collaboration with the family and collaterals.
• Develop an initial safety/crisis plan.
