Job summary

Child & Family Services is seeking a Care Coordinator to coordinate services identified in the ICP, reporting to the Senior Care Coordinator and CSA Program Director. The role includes conducting home-based assessments and coordinating with CPT teams to implement youth- and family-centered care plans.

The position involves ongoing family education, advocacy, and collaboration with state agencies, schools, and other providers to ensure timely service delivery.

Responsibilities

• Conducting a comprehensive, home-based assessment including the CANS and other tools as needed, conducted in the youth's home or another location.
• Identifying with the family the appropriate members of the CPT.
• Developing and implementing a youth- and family-centered ICP with the family and collaterals.
• Developing a risk management/safety plan with the family and collaterals.
• Maintaining regular contact with the family, youth (where appropriate), and other relevant persons in the youth's life.
• Facilitating CPT meetings.
• Maintaining face-to-face contact with the youth and family as determined.
• Identifying and developing with the family natural supports.
