Job summary

Bay State Community Services in Plymouth, MA seeks an Intensive Care Coordinator to lead wraparound care for youth with complex behavioral health needs. You will coordinate teams, assess family needs, and develop care plans that reflect strengths and goals.

Qualifications

  • Bachelor’s degree in Human Service field with relevant experience in navigating child/family systems.
  • Bachelor’s degree in other fields with significant related experience in behavioral health navigation.
  • Associates or high school with substantial relevant experience in navigating family systems.
  • Master’s degree in a mental health field from an accredited college/university.
  • Willing to pursue MA CANS certification with training provided.
  • Bilingual abilities in Spanish, Portuguese, or Haitian Creole are desirable.

Responsibilities

  • Facilitate Care Planning Team meetings with youth, families, providers and supports.
  • Conduct comprehensive assessments to identify family strengths and needs.
  • Develop, implement, and monitor individualized care plans.
  • Coordinate access to community-based services and supports.
  • Advocate for youth and family voice in planning and delivery.
  • Collaborate with state agencies, schools, providers and partners.
  • Maintain accurate documentation per program standards.

Education

Bachelor’s degree in Human Service field
Bachelor’s degree in non Human Service field with 1+ year relevant experience
Associate’s degree or High School Diploma with 5+ years relevant experience
Master's Degree in a Mental Health Field
Willing to become certified MA CANS

About The Role

The ICC serves as the primary point of coordination for youth with Serious Emotional Disturbance (SED) and their families. Using the Wraparound model, you will bring together providers, natural supports, and community resources to build a comprehensive care plan that reflects the family’s strengths, needs, and goals.

This is a dynamic, relationship-focused position ideal for someone who excels at communication, problem-solving, and advocacy.

Requirements
Qualifications
  • Bachelor’s degree in Human Service field and minimum 1 year relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems; OR
  • Bachelor’s degree in non Human Service field and more than 1 year relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems; OR
  • Associate’s degree or High School Diploma and minimum of 5 years relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems
  • Master's Degree in a Mental Health Field including but not restricted to Counseling, Family Therapy, Social Work, Psychology etc. from an accredited college/university
  • Willing to become certified in MA Child And Adolescent Needs and Strengths (CANS), program will provide training.
  • Bi-lingual in Spanish, Portuguese, and Haitian Creole desired
