Job summary

Child & Family Services, Inc. seeks a Care Coordinator to coordinate all services identified in the ICP.

The role reports to the Senior Care Coordinator and the CSA Program Director and requires a Massachusetts driver’s license and Massachusetts CANS certification. You will conduct home-based assessments, develop youth- and family-centered ICPs with families and collaterals, maintain regular contact, arrange CPT meetings, coordinate with state agencies and schools, and prepare reports and

  • Experience navigating child/family–serving systems and advocating for families in behavioral health.
  • Massachusetts driver’s license and automobile.
  • Certified in Massachusetts CANS.
  • Evening/weekend availability.

Responsibilities

  • Conduct a comprehensive home-based assessment including CANS and other tools.
  • Develop and implement a youth- and family-centered ICP with family and collaterals.
  • Maintain regular contact with family, youth (where appropriate), and CPT members.
  • Coordinate services with state agencies, the school system, primary care, and other providers.
  • Prepare reports, authorizations, ICPs, and evaluation data.
  • Provide after-hours phone support for urgent crises.

SummaryThe Care Coordinator is responsible for coordinating all services and supports identified in the ICP. They are responsible to the Senior Care Coordinator and the CSA Program Director.$1 per hour Language Differential! Therefore, final offer amounts may vary from the amount stated.Job Responsibilities & Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.Conducting a comprehensive, home-based assessment inclusive of the CANS and other tools as determined necessary, which occurs in the youth’s home or another location.Identifying with the family-appropriate members of the CPTDeveloping and implement a youth- and family-centered ICP in collaboration with the family and collateralsDeveloping a risk management/safety plan in collaboration with the family and collateralsMaintaining regular contact with the family, youth (where appropriate), and other relevant persons in the youth’s life (collaterals)Facilitating CPT meetingsMaintaining face-to-face contact with the youth and family, as determined by the youth and family and members of the CPTIdentify and develop with the family natural supportsAssisting with system navigationProviding family education, advocacy, and supportIdentifies and actively assists the youth and family to obtain and monitor the delivery of available services including medical, educational, social, therapeutic, or other servicesMonitoring, reviewing, and updating the ICP to reflect the changing needs of the youth and familyProvide after-hours phone support to address urgent child or family crises. Care Coordinators must have a valid Massachusetts driver’s license and an automobile. Care Coordinators must be certified in the Massachusetts CANS.
