• Associate’s degree in human services or related field, or a high school diploma with 3 years’ related experience.
• Proficiency in Microsoft Office Suite and record-keeping.
• Ability to communicate clearly with clients and partners; comfortable presenting to groups.

Responsibilities

• Gather intake information for funding sources and service delivery.
• Inform residents about available resources and services.
• Distribute information on community needs and issues.
• Provide support and stabilization services to individuals and families.
• Coordinate with partners to connect clients with resources.
• Maintain confidential client records.
• Connect clients with seminars and educational opportunities.
• Represent the agency at community events and staffing information tables.
• Provide case management, including needs assessment and service planning.
• Assist clients with applications and documentation for benefits.
• Follow up to confirm access and address participation barriers.
• Perform other duties as assigned.
