Qualifications

• Associate’s degree in human services, social sciences, or related field.
• One year of experience in a related field.
• Alternatively, a High School Diploma with three years of related experience.

Responsibilities

• Gathers and compiles intake information required by funding sources for service delivery.
• Informs area residents of available resources and services.
• Distributes written information related to community needs and issues.
• Provides support and stabilization services to individuals and families.
• Coordinates with community partners and service providers to connect clients with resources that meet their needs.
• Maintains accurate and confidential client records.
• Connects clients with seminars, workshops, and educational opportunities based on their needs and interests.
• Represents the agency at community events by staffing information tables, sharing resources, and connecting residents with services (evenings/weekends).
• Provides case management, including assessing client needs, developing service plans, coordinating referrals, and monitoring progress.
• Assists clients with applications and documentation needed to access benefits and community resources.
• Follows up with clients and service providers to confirm access to services and address barriers to participation.
• Must present a professional image in conduct, attitude, and attire.
• Satisfactory completion of mandatory trainings.
• Additional duties as assigned.
