Job summary

Rosecrance Ware Center in Rockford, IL is seeking dedicated professionals to provide post-crisis stabilization and connect clients to ongoing care tailored to each individual's needs. You will conduct formal assessments, determine treatment needs, and develop safety plans with clients and families, coordinating with Mobile Crisis Response to ensure timely support.

Schedule: Tuesday–Saturday, 10:00 am–6:30 pm, onsite at Rosecrance Ware Center.

Qualifications

• Bachelor’s degree or equivalent experience required for role.
• Must meet MHP qualification per IL Rule 132.
• Strong written and verbal communication skills.
• Ability to establish rapport with clients in crisis.
• Computer proficiency and EHR documentation ability.
• Independent worker with teamwork capability.
• Ability to manage multiple deadlines and documentation.
• Valid driver’s license and ability to transport clients.
• Commitment to Rosecrance mission and values.

Responsibilities

• Provide crisis stabilization services in community settings.
• Conduct formal assessments and identify treatment needs.
• Develop safety plans with clients and families.
• Collaborate with Mobile Crisis Response for ongoing support.
• Complete documentation in electronic health records.
• Maintain caseload with focus on linking clients to long-term care.
• Ensure discharge planning and interagency communication.
• Coordinate care with outside providers to ensure accurate discharge planning.
