Job summary

Qualifications

• Education: High School Diploma or GED required.
• Bilingual English/Spanish; strong written and verbal communication.
• Experience in case management or medical office settings preferred.

Responsibilities

• Plan, facilitate and coordinate case management for Mobile Clinic patients.
• Answer referrals, schedule medical and mammogram appointments.
• Coordinate care and maintain patient contact via phone and in person.
• Input and track information in EMR systems; communicate with providers.
