Job summary

UMass Memorial Health is seeking a Psychiatric Case Manager to provide psychosocial assessments and coordinate care for patients in crisis on campus and mobile units. You will determine appropriate levels of care and collaborate with families and community resources to ensure seamless transitions.

Required: Master's in Human Services or equivalent, and license eligibility (LICSW/LISW/LMHC). Excellent communication, risk assessment, discharge planning, and teamwork are essential.

Qualifications

• Master's Degree in Human Services or equivalent.
• License eligibility upon hire (LICSW, LISW, or LMHC).
• Preferred LICSW, LICSW, or LMHC licensure.
• 1-3 years experience in Psychiatry/Mental Health.
• Psychosocial assessment, community resources, crisis intervention, and discharge planning.
• Excellent written and verbal communication skills.
• Basic computer skills.

Responsibilities

• Provide psychosocial history and mental status assessments in psychiatric crisis.
• Assign clinical level of care and provide referrals to inpatient, outpatient or diversion programs.
• Maintain a safe therapeutic environment and assist in psychiatric emergencies.
• Triages calls of psychiatric emergencies and coordinate interventions and referrals.
• Coordinate case management and access to psychiatric treatment and social services.
• Communicate with outpatient treaters to coordinate treatment planning and referrals.
• Ensure compliance with managed care requirements and document accordingly.
• Maintain service logs and codes for billing; assist with scheduling.
• Engage in continuing education to maintain licensure and keep clinical skills updated.
• Maintain BLS certification and psychiatric competencies.
