Job summary

Connections Health Solutions is seeking a Case Manager II to support individuals in crisis through psychoeducation, assessment, and recovery planning. You will coordinate care with a multidisciplinary team, facilitate discharge planning, and connect clients to community resources.

The role emphasizes collaboration, documentation in the EHR, and ensuring safety while meeting clinical needs. A BS in behavioral health and Virginia CSAC licensure are required.

Qualifications

• Bachelor’s degree in behavioral health or related field.
• Knowledge of Virginia behavioral health system and resources.
• CSAC licensure by Virginia Board of Counseling.
• Vaccination policy compliance and documentation.
• DBHDS background check and DSS Central Registry check.

Responsibilities

• Educate individuals on consents and disclosures.
• Participate in Interdisciplinary Treatment Team meetings.
• Develop and implement person-centered Treatment Plans.
• Collect collateral and assess treatment needs for discharge.
• Coordinate care and referrals with facility and external providers.
• Identify gaps and recommend services for discharge.
• Maintain accurate clinic records and EHR entries.
• Understand Virginia involuntary commitment processes.
