Job summary

Connections Health Solutions is seeking a Case Manager to support admission intakes, treatment planning, and discharge planning for individuals in crisis. You will collaborate with the treatment team, coordinate care with partners, and maintain detailed EHR documentation.

Qualifications

• Bachelor’s degree in behavioral health or related field.
• Knowledge of the Commonwealth of Virginia behavioral health system.
• Qualified Mental Health Professional (QMHP).
• Vaccination policy compliance and criminal background checks per policy.

Responsibilities

• Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
• Participates regularly in Interdisciplinary Treatment Team meetings.
• Develops and implements an individualized person-centered Treatment Plan for brief intervention.
• Coordinates care and discharge planning with facility staff and external providers.
• Maintains the clinic record and documents in the EHR.
