Job summary

The Providence Center is seeking a qualified behavioral health professional to join a multidisciplinary team providing psychosocial assessments and crisis evaluation for consumers in our care and in community settings.

You will perform comprehensive assessments, collaborate with other providers, and develop clear, consumer-centered care plans while maintaining strict confidentiality and professional conduct.

Qualifications

• Master’s degree in Counseling or Social Work or RN with licensure; QMHP eligibility.
• Experience with crisis assessment and behavioral health settings is preferred.
• Maintain confidentiality and follow Health Information Services policies.

Responsibilities

• Perform a comprehensive psychosocial assessment of assigned consumers.
• Treat consumers with respect and professionalism during crises.
• Assess clinical emergencies, including suicide risk and danger to others.
• Complete crisis and initial assessments and write integrated summaries.
• Develop and implement a client-centered care plan with consumer input.
• Maintain accurate records and required reports in compliance with policies.
• Collaborate with providers and community agencies in the network of care.
