Job summary

Horizon House, Inc. invites applications for an Associate Mental Health Specialist to join the ACT Alliance team in Georgetown, Delaware. The role provides rehabilitation and support services, crisis intervention under clinical supervision, and collaboration with families and treatment teams.

The position emphasizes treatment planning, coordination with community agencies, and response to emergencies, including on-call crisis coverage and local travel as needed.

Qualifications

• Bachelor’s degree in social work, psychology, rehabilitation or mental health counseling, or another related human services field.
• Knowledge of mental health and substance abuse systems and supports.
• Ability to work independently.
• Good communication skills both oral and written.
• Possess a valid driver’s license with an acceptable driving record.

Responsibilities

• Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
• Provide service coordination (case management) for an assigned group of participants including coordinating and monitoring the activities of the individual’s treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising the overall treatment goals and plans in collaboration with the participant and the Team.
• Provide on-call crisis intervention covering nighttime hours.
• Provide symptom education to enable participants to identify their mental illness symptoms.
• Coordinate with state vocational rehabilitation and other employment services.
• Provide treatment, rehabilitation and support services with some interventions directed or performed by staff with specialty training and skills (e.g. vocational specialist).
