Job summary

Horizon House, Inc. is seeking a Full-Time Crisis Worker to provide residential support to individuals experiencing a mental health crisis who are not at immediate risk.

The role helps participants learn to live independently in the community and coordinates access to specialized services. Responsibilities include admission orientation, monitoring progress toward personal and treatment goals, assisting with daily living tasks, and communicating with the care team.

Qualifications

• High school diploma or GED required; 3 years mental health direct care experience preferred.
• Knowledge of mental health systems and supports.
• Familiarity with privacy regulations and access/disclosure rules.
• Strong written and oral communication skills.
• Must have a valid driver’s license and driving record meeting company standards.

Responsibilities

• Admit assigned participants, complete intake paperwork, and orient to facility routines.
• Observe data on participants and note changes in mental/physical health.
• Support participants toward independence; counsel on goals, values, responsibilities, and future planning.
• Assist with petty cash accounting and routine expenditures.
• Collaborate with rehabilitation/treatment team, case managers, and families to coordinate care and plan conferences as needed.
• Report repairs, safety concerns to supervisor; participate in routine cleaning and maintenance.
• Assist participants with ADL skills, household tasks, finances, and related activities.
