Job summary

La Frontera EMPACT - Suicide Prevention Center is seeking a Children’s Case Manager in San Tan Valley, AZ. The role is full-time and requires scheduling flexibility to meet client and family needs.

You will provide direct crisis intervention, support, transportation and education to families and children, and complete family needs assessments with referrals to partners and community services as needed.

Qualifications

• Bachelor’s degree or higher and 1 year of behavioral health experience OR associate’s degree with 2 years of behavioral health experience OR high school diploma with 4 years of behavioral health experience.
• Experience with the Child and Family Team process.
• Fingerprint clearance card or eligible to obtain one.
• Valid AZ driver’s license with no major infractions in past 3 years.
• Bi-lingual (Spanish) preferred.
• Bi-lingual Diff and Rural Diff pay rate per hour.
• Staff must be at least 21 years old.

Responsibilities

• Provide direct service crisis intervention, support, transportation and education to families and children.
• Conduct family needs assessments and make referrals to collaborating partners and community services.
• Ensure continuity of care and coordinate communications with other departments or collaborators.

Education

Bachelor’s degree or higher + 1 year behavioral health experience

Associate’s degree + 2 years behavioral health experience

High school diploma + 4 years behavioral health experience
