EMPACT-SPC/La Frontera Arizona is hiring a full-time Children's Case Manager for our San Tan Valley location. The role involves crisis intervention, transportation, and education for families and children, with scheduling flexibility to meet client needs.

The candidate will conduct family needs assessments, coordinate referrals, and ensure continuity of care across departments and partners, while maintaining professional conduct and ethics.

Qualifications

• Bachelor’s degree or higher with one year behavioral health experience, or an associate’s degree with two years.
• High school diploma with four years behavioral health experience.
• Experience with the Child and Family Team process.

Responsibilities

• Provide direct service crisis intervention, support, transportation and education to families and children.
• Complete family needs assessments and make referrals to partners and community services.
• Coordinate communications with other departments or collaborators to ensure continuity of care.
