Job summary

La Frontera EMPACT - Suicide Prevention Center is looking for a High Needs Case Manager in San Tan Valley, Arizona. This role involves direct service in crisis intervention for children and families, managing a caseload of 15 very active clients.

The ideal candidate will possess a Bachelor’s degree in a behavioral health field and have previous hands-on experience with children. The position offers generous benefits, including medical, dental, vision plans, and a 403(b) retirement plan.

Qualifications

• Bachelor's Degree in Behavioral Health related field preferred or meets BHT requirements.
• Behavioral health experience is required.
• Previous experience working with children is preferred.
• Knowledge of the CFT process and Arizona 12 Principles.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services.
• Facilitate and schedule follow-up on family, youth, and adult team meetings.
• Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact.
• Ensure continuity of care in providing comprehensive services.
