Clinical & Support Options, Inc. (CSO) in Gardner, MA is seeking a Case Manager Supervisor for the Emergency Services Program (ESP).

The role oversees Crisis Case Managers, ensures timely client engagement, and provides direct CM to Crisis clients to support stabilization and community linkage. You will coordinate linkages to CSO, behavioral health and SUD providers, medical services, and wellness activities, while maintaining a person-first approach to care and collaborating closely with the

Qualifications

• Bachelor's or Master's degree in a behavioral health discipline or equivalent.
• Peer experience may be considered as applicable.
• Experience working with adults with mental health challenges including navigating behavioral health system.

Responsabilités

• Provide programmatic and administrative supervision for Crisis Case Managers and engage clients within 24 hours.
• Supervise case management to support crisis stabilization and linkages to CSO and other providers.
• Provide direct case management to Crisis clients as needed and assist with housing, transportation, and access to supports.
• Engage crisis clients by phone and in person; be responsive and person-first in care.
• Consult with clinical supervisor on safety issues and transitions; follow up until clients are engaged with community supports.
