Clinical and Support Options, Inc. (CSO) seeks a Case Manager Supervisor for the Emergency Services Program in Leominster, MA. The role oversees crisis case management, engages clients, and ensures timely contact and linkage to supports to promote stabilization.

Qualifications

• Bachelor's or Master's degree in a behavioral health discipline or equivalent.
• Peer experience may be considered.
• Experience with adults with mental health challenges and navigating the behavioral health system.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide programmatic and administrative day-to-day supervision for Crisis Case Managers in CM identification, crisis stabilization, and linkages to service supports.
• Engage Crisis clients and ensure Case Managers engage clients within 24 hours.
• Provide individualized support based on each client's profile and needs.
• Provide outreach to clients by telephone and in person and through their support network.
• Clarify transition needs, gaps in services and supports and assist in locating housing and transport.
• Assist clients in identifying and using natural supports including family and friends.
• Be responsive, flexible, timely, and client-centered in service delivery.
• Consult with clinical supervisor on safety issues, barriers to transition, and interventions.
• Remain engaged with clients after initial Crisis encounter until stable and connected to community supports.

Job description

CSO is looking for a Case Manager Supervisor for our Emergency Services Program(ESP)in Leominster.

This position will supervise case management services for adult Crisis clients and provide direct case management services to Crisis clients. Case management provides follow up support to individuals evaluated by Crisis Services and who remain in the community after a Crisis assessment.

As a Case Manager Supervisor:

You Will:

• Provide programmatic and administrative day-to-day supervision for Crisis Case Managers in identifying clients needing case management (CM), focusing support on crisis stabilization, and facilitating linkages to service supports including CSO, other mental health and SUD providers, medical providers and wellness activities
• Engage Crisis clients, and ensure Case Managers engage clients either at the time of the initial Crisis encounter or within 24 hours through the client's preferred method of contact
• Provide individualized support based on person's unique profile including the individual's support system, provider recommendations and support needs identified by the individual
• Provide outreach to clients by telephone and in person and through their support network
• Provide assistance in clarifying transition needs, gaps in services and supports
• Assist in locating housing, transport to appointments, and accessing supports for stabilization needs
• Assist clients in identifying and using natural supports including family and friends
• Be responsive to client requests, flexible and timely in providing services, creative in finding solutions, and dedicated to a person-first approach to care
• Consult with clinical supervisor as needed including safety issues, barriers to transition, and questions about effective interventions, and focusing care on stabilization
• Remain engaged with individual after the initial Crisis encounter and until client is stable and engaged in appropriate community supports

Your Background:

• Bachelor's or Master's degree in a behavioral health discipline or related field or equivalent combination of high school diploma or Associate's degree and applicable experience
• Peer experience may be considered as applicable
• Experience working with adults with mental health challenges including experience navigating the behavioral health system
• Valid driver's license and reliable transportation
