Job summary

BestCare is seeking a Case Manager to coordinate care, manage data, and provide direct clinical services within the crisis stabilization program. You will build relationships with clients and with community providers to ensure access to services and adherence to program outcomes.

The role includes assisting clients with resources, documenting in the EHR, and collaborating with a multidisciplinary team to support treatment plans and recovery goals while meeting regulatory standards and payor

Qualifications

• Bachelor’s degree in behavioral science or related field required (Level II).
• Associates degree in behavioral science plus 4 years in social services or counseling (Level I).
• Experience with individuals with mental illness (SPMI).
• Must have maintained recovery in substance-use disorders if applicable.

Responsibilities

• Coordinate services and benefits for crisis stabilization program participants.
• Develop and maintain relationships with clients and community providers.
• Manage data to track contract metrics and program outcomes.
• Provide direct clinical services, including periodic substance use assessments.
• Document in the EHR and ensure timely filing within regulatory standards.
• Assist clients with accessing resources and entitlement programs.
• Support treatment plans and recovery goals through ISSP/Individual Services & Support Plan.
