Job summary

Community Bridges, Inc. seeks a Familiar Face Case Manager to coordinate care and discharge planning for individuals using crisis services. The role focuses on preparing patients for appropriate services and coordinating with involved parties to reduce barriers to treatment.

Reports to the Clinical Director. Requires a high school diploma or GED; an associate degree in behavioral health is preferred; relevant experience in mental health and substance use is needed.

Qualifications

• High school diploma or GED required.
• Associates degree in a field related to behavioral health preferred.
• Minimum six months of experience in multiple settings and levels of care, including mental health and substance use, required.
• One or more years of experience preferred.
• Minimum six months of recovery from substance use and/or mental health disorders preferred.
• Current driver’s license (valid and in good standing).
• 39-month Motor Vehicle Record (MVR) required.
• Behavioral Health Technician (BHT) certification per policy.

Responsibilities

• Coordinate care and discharge planning for individuals utilizing crisis services.
• Assist patients in obtaining clinically appropriate services and coordinate with involved parties.
• Reduce barriers to effective treatment and prepare patients for appropriate services.
• Report to the Clinical Director.
