Job summary

BestCare is seeking a Case Manager to coordinate services, manage data, and provide direct clinical services within the crisis stabilization program. The role emphasizes building strong relationships with clients and community providers to promote crisis stabilization.

Key duties include assisting clients with resources, supporting treatment plan goals, and maintaining timely documentation in the EHR, including potential Payee responsibilities after training.

Qualifications

• Bachelor’s degree in behavioral science required (Level II) or Associates with four years in social services or counseling (Level I)
• Experience with individuals with severe and persistent mental illness (SPMI)
• For substance-use recovery, must have maintained continuous recovery for two years at hire

Responsibilities

• Works as part of a treatment team on the Mental Health Team
• Assists clients with accessing resources, benefits, and entitlement programs
• Provides support to clients to achieve treatment plan goals (ISSP/Individual Services & Support Plan)
• Develops collaborative relationships with community partners (e.g., Self-Sufficiency, Public Health, Mosaic Medical)
• Assists clients in ACT-Assertive Community Treatment with Personal Recovery Plans
• May provide Payee services for Social Security disability after training
• Provides transportation for clients when appropriate
• Participates in home visits and welfare checks with clinician supervision
• Leads psycho-educational or skill-building groups when appropriate
• Maintains timely clinical documentation in the EHR within 72 hours of service
• Facilitates services at BestClub and coordinates with clients not in BestClub
• Participates in trainings and mentors new staff
• Models counselor behavior and supports staff training
• Engages clients with compassion and supports ongoing treatment engagement
• Performs other related duties as assigned
