Piedmont Community Services is seeking a Care Coordinator to support individuals in crisis, coordinating services and communicating with the crisis team, the emergency department, and local jails. A bachelor's degree in human services is preferred, with at least one year of related experience and a valid driver’s license.

Responsabilidades

• Triage incoming phone calls for crisis cases and coordinate care.
• Link individuals and families to appropriate team members and services.
• Facilitate discharge planning to higher or lower levels of care as directed by Crisis Clinician.
• Ensure all parties are informed of the status of individuals receiving care at the Lighthouse, ED, and local jails.
