Job summary

Milwaukee County is seeking a Crisis Care Coordinator (Youth) within Behavioral Health Services to work with youth and their families to identify and access community resources. The role follows up with families within two business days of referral to review programs, obtain consents, and craft individualized plans between referral and enrollment in Wraparound programs.

The position requires a bachelor’s degree in Human Services, Psychology, or Social Work and at least one year of relevant

Qualifications

• Current resident of Wisconsin.
• Possession of a valid Wisconsin Driver’s License and insured personal vehicle.
• At least one year of professional experience with providing mental health services to youth.
• Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets.
• Willing and able to travel up to 75% of the time.

Responsibilities

• Deliver Crisis Care Coordination until other services can be obtained or the youth has stabilized; assist with benefits, linkage to services, crisis planning.
• Meet with youth and families weekly to provide direct services until enrolled or linked to wraparound supports.
• Develop individualized care plans measurable under state guidelines and document progress notes.
• Collaborate with other providers and coordinate services as needed to support DHHS and Wraparound operations.
• Perform other duties as assigned.
