Responsibilities

• Facilitate admission intakes, treatment planning, and discharge planning.
• Coordinate with interdisciplinary treatment team and community partners.
• Maintain clinic records and ensure proper signatures.
• Monitor Title 36 involuntary commitment processes and hearings.
• Engage in ongoing supervision and training to maintain competency.
• Perform other duties as assigned.
