Job summary

RI International is seeking a Care Coordinator in Columbus, OH. The role supports admission, discharge, outreach, engagement, recovery and case management within a Crisis setting. You will coordinate care and resources across internal teams and community providers, advocating for participants’ needs and rights.

Key duties include documenting information for coordination, adhering to regulations, and collaborating with multidisciplinary teams. A background in behavioral health is required.

Qualifications

• Bachelor’s Degree in a behavioral health field with 1+ year of behavioral health experience; or
• Bachelor’s Degree in any field with 2+ years of behavioral health experience; or
• HS diploma with 4+ years of behavioral health experience.

Responsibilities

• Coordinate care and resources for individuals across internal and community services.
• Advocate for needs and rights of individuals and assist in daily living activities.
• Document information to support care coordination and compliance.
• Collaborate with multidisciplinary teams to ensure timely services.
