Job summary

Recovery Innovations in Columbus, OH is seeking a Care Coordinator to facilitate the quality and improvement of a person’s experience of care, including admission, discharge, outreach, engagement, recovery, transition and case management.

The shift is As Needed, 7:00PM–7:30AM at the Crisis Care Center, with a wage of $19.00–$23.00/hour. Strong communication, computer skills, and de-escalation training are required for this role.

Qualifications

• Bachelor’s degree in behavioral health field with 1 year of behavioral health experience, or
• Bachelor’s degree in any field with 2 years of behavioral health experience, or
• High School Diploma plus 4 years of behavioral health experience.

Responsibilities

• Guest Care & Advocacy: coordinates care and resources; advocates for needs and rights; assists daily living and community linkages.
• Compliance & Risk Management: documents information to coordinate care; follows regulations and policies.
• Cross Functional Collaboration: coordinates multidisciplinary meetings; partners with providers to achieve wellness.
• Additional Duties: performs other related duties as assigned.
