Job summary

RI International is seeking a Care Coordinator for the Crisis - Franklin County Crisis Care Center in Columbus, OH. The role supports the 7:00 PM to 7:30 AM shift on an as-needed basis and involves coordinating care, advocacy, and community linkages for individuals on their recovery journey.

The position requires a behavioral health background with 1–4 years of relevant experience or an equivalent combination of education and experience, along with strong communication and computer skills.

Qualifications

• Bachelor’s in behavioral health with 1 year of behavioral health experience, or a BA with 2 years of behavioral health experience; or high school diploma with 4 years of behavioral health experience.
• Ability to provide courteous, concise, informative assistance with a positive tone.
• Excellent computer skills, including Microsoft Office.

Responsibilities

• Coordinate care of services and resources across internal and community networks.
• Advocate for the needs and rights of individuals.
• Assist with daily living activities and establish linkages for education, support and independence.
• Document information to support communication and compliance.
• Coordinate multidisciplinary team meetings and partner with other providers.
• Perform other related duties as assigned.
