Job summary

Hamilton County is seeking a deflection peer navigator to collaborate with first responders and crisis counselors to provide crisis intervention and guide individuals and families toward mental health, substance use services, homelessness resources, and long-term care.

You will maintain records in the Cordata system, coordinate with partners, and support program goals to reduce involvement with the criminal justice system by connecting participants to appropriate care services.

Qualifications

  • Bachelor’s degree in social work, sociology, psychology, or a related field.
  • One (1) or more years of experience in social services, case management, or a related capacity.
  • Or equivalent combination of training and experience demonstrating knowledge of social services and case management practices.

Responsibilities

  • Assist on a variety of deflection initiatives to help navigate individuals out of the criminal justice system and into care.
  • Identify gaps and barriers to participant services and connect them with partners and resources.
  • Maintain accurate participant records in the Cordata system while ensuring confidentiality.
  • Execute projects related to co-responder deflection and communicate with grant stakeholders.
  • Develop collaborative relationships with law enforcement and court partners in Cincinnati and surrounding counties.
