Job summary

Integrated Services for Behavioral Health (ISBH) seeks an Engagement Care Coordinator to support youth enrolled in OhioRISE, coordinating care and guiding families through discharge planning and follow-up referrals.

The role involves collaboration with families across home, school, and community settings, conducting crisis plans, and facilitating Child and Family team meetings until a warm handoff to ongoing care coordination. Strong organizational and communication skills are essential.

Qualifications

• Experience delivering services and/or support to children and families in behavioral health or related public sector human services.
• Knowledge of coordinating care and supporting families in home, school, and community settings.
• Comfort with virtual and in-person meetings and using internet-based office environments.

Responsibilities

• Identify care coordination needs with families to meet program outcomes.
• Define cultural factors influencing strengths and family interaction styles.
• Coordinate family-based services in home, school, and community.
• Ensure services on care plans are appropriate and least restrictive.
• Engage with families to support successful plan implementation and team integration.
• Develop partnerships with community resources to meet diverse needs.
• Maintain documentation and participate in program planning and evaluation.
