Job summary

The Centers’ Glick Recovery Campus seeks a Coordinator, Continuous Care to streamline patient flow from admission through discharge. You will serve as a central hub, coordinating administrative and clinical staff in a high-acuity setting.

You will support check-in, registration, eligibility, and care coordination, ensuring timely access to internal resources. Strong communication, calm judgment, and EHR proficiency are essential for patient safety and service delivery at all times.

Qualifications

• Minimum HS diploma; Bachelor's preferred.
• 3+ years in behavioral health or similar fast-paced setting.
• 2+ years in patient access, registration or scheduling in high-volume env.
• Experience with EHR systems and coordinating across multidisciplinary teams.

Responsibilities

• Client Access and Care Coordination: manage admission, registration, eligibility, enrollment, and care coordination across the campus.
• EHR and Data Integrity: enter and verify patient and payer information in EPIC and other systems; perform reminders.
• Scheduling and Utilization Management: oversee provider scheduling and analyze utilization.
• Cross-Team Collaboration: collaborate with clinical, supervisory, and clerical staff and support during vacations or call-offs.
• De-escalation and Safety: participate in de-escalation and crisis response as trained.
• Financial and Compliance: collect co-pays and ensure HIPAA compliance.
