Job summary

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read and understand procedures and manuals.

Responsibilities

• Assist in planning and conducting individual and group activities.
• Support communication between staff, residents, families and the public to meet residents' needs.
• Develop monthly activity calendars and maintain attendance records.
• Assist with discharge planning and activity care plans as requested by the Activities Director.
• Arrange transportation for residents for outings, appointments, or discharge.
• Encourage resident participation in hobbies, crafts and reading.
