Job summary

Qualifications

• Six months of experience as County Caseworker 1 or related intern program.
• Bachelor's degree with a social welfare major or equivalent with social science credits.
• One year of professional social casework experience or equivalent.
• Educational requirements waived when within two months of graduating.

Responsibilities

• Plan Development: Prepare individualized service plans outlining goals and needed supports.
• Benefit Assistance: Guide applications for financial and medical benefits.
• Provider Monitoring: Review residential and day program services for implementation of supports.
• Care Documentation: Maintain thorough records supporting care and advocacy.
• Progress Coordination: Lead meetings to evaluate client progress toward goals.
• Crisis Intervention: Respond to emergency situations as assigned.
