Job summary

The Commonwealth of Pennsylvania is seeking a County Caseworker 1 for Northumberland County's Intellectual/Developmental Services Program to support individuals in meeting personal goals and gaining access to services.

You will gather information, develop service plans, assist with benefits, coordinate progress, coordinate community supports, and maintain documentation; telework is not available. Hours are 8:30 AM–4:30 PM, Monday to Friday.

Qualifications

• Two years as County Social Services Aide 3 and 12 college credits in social sciences.
• Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences.
• Equivalent combination of experience and training with 12 credits and one year of paraprofessional case management.

Responsibilities

• Plan development: prepare individualized service plans with goals and needed services.
• Benefit assistance: help individuals apply for available benefit programs.
• Progress coordination: lead meetings to review consumer progress toward goals.
• Supportive services: arrange budgeting guidance, social skill development, and daily living support.
• Documentation management: maintain records to support continuity of care and advocacy.
• Crisis response: provide immediate assistance during emergencies under supervision.
