• Must meet educational requirements within two months of graduation.

Responsibilities

• Plan individualized service plans describing goals and needed services.
• Assist individuals in applying for available benefit programs.
• Lead meetings reviewing consumer progress in goals.
• Arrange community supports like budgeting guidance and daily living help.
• Maintain records to support continuity of care and advocacy.
• Provide immediate crisis assistance as directed.
