Job summary

The Commonwealth of Pennsylvania seeks a County Caseworker 1 for Fayette County MH/ID to help individuals access resources and supports toward safer, more independent lives.

You will plan and coordinate services, guide person-centered discussions, monitor progress, and link clients to benefits. Strong communication and organizational skills are essential; telework is not available. Email notices will follow.

Qualifications

• Two years of experience as County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
• Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Responsibilities

• Plan Development: Create individualized service plans reflecting a person’s needs and preferences.
• Service Coordination: Connect individuals to providers, community supports, and resources.
• Person-Centered Support: Lead planning discussions that emphasize personal choice and control.
• Service Monitoring: Ensure services align with the service plan.
• Health and Safety Oversight: Monitor well-being of individuals receiving supports.
• Resource Linking: Direct individuals to benefits, services and information.

Education

Experience as C.S.S. Aide 3

Bachelor's degree with 12 credits in social sciences

Equivalent combination + paraprofessional exp.

Graduation within 2 months of degree
