The State of Colorado is seeking a dedicated case management professional for the Colorado Department of Corrections at Buena Vista. You will conduct daily case management to support offender re-entry, using motivational interviewing and CTAP to coordinate services, assess risks, and develop individualized plans.

• Three years of direct case management with high-need populations.
• Experience in corrections or human services environments.
• Combination of education and experience as listed in MQs.

Responsabilidades

• Meet with offenders to guide them per CDOC needs and regulations.
• Deliver case management using motivational interviewing and CTAP.
• Coordinate access to work, education, treatment and programs.
• Monitor progress and update assessments regularly.
• Assist offenders in transition planning and community reintegration.
• Maintain multidisciplinary communications and crisis intervention as needed.
