Job summary

The State of Colorado's Department of Corrections, Buena Vista Correctional Complex, seeks a dedicated Case Manager to perform daily case management focused on offender re-entry and continuity of care. You will work with an advocacy-based team to guide individuals through intake to release under CDOC regulations.

You will meet with offenders, coordinate access to work, education, and treatment, monitor progress, and prepare plans for parole boards, while maintaining multidisciplinary

Qualifications

• Three years direct experience with high-need clients in correctional or human services.
• Experience with offender management processes (ICC, classifications, parole planning).
• Combination of education in Criminal Justice, Social Work, Sociology or Psychology.

Responsibilities

• Meet with offenders to guide them per CDOC needs and regulations.
• Deliver case management using motivational interviewing and CTAP.
• Coordinate access to work, education, treatment and programs.
• Develop parole plans and assess risk with parole boards.
• Monitor progress and complete assessments per policy.
• Maintain multidisciplinary communication with offenders and staff.
