Job summary

Maine Correctional Center is hiring a Correctional Care and Treatment Worker (CCTW) to serve as the primary case manager for residents' treatment plans. The role partners with security, behavioral health, and other staff to create individualized reintegration plans and to provide coaching to support institutional adjustment and needs.

The position involves non-clinical programming in areas such as domestic violence, cognitive behavioral programs, and pro-social communication, with collaboration

Qualifications

• Three-year combination of education, training, and/or experience providing knowledge of case management principles and practices and/or correctional care and treatment programs.

Responsibilities

• Gather and review background information and conduct interviews to orient residents and perform assessments.
• Develop and update case management plans with referrals to programs, jobs, education and vocational training.
• Track progress of case-management plans and keep the team informed.
• Maintain thorough documentation including case plans and progress notes.
• Meet with residents regularly to provide support, coaching, and assist with reentry plans.
• Collaborate with team to modify plans to ensure safety and success.
• Facilitate non-clinical programs and interventions with individuals and groups.
