Job summary

Community Action Pioneer Valley, Inc. seeks a Coordinated Entry (CE) Coordinator to lead the Three County CoC CE system across Berkshire, Franklin, Hampshire Counties and North Quabbin.

The role focuses on equity, trauma-informed practice, and ensuring access to housing resources through HMIS data, training, and system-wide improvements. The CE Coordinator works with providers, people with lived experience, and government partners to identify barriers, coordinate case conferencing, and improve

Qualifications

• 4 years' experience in human services working with low-income individuals or families and vulnerable populations.
• Experience in an office setting including data entry.
• Knowledge of social services and navigating resources.

Responsibilities

• Manage day-to-day operations of Coordinated Entry system across multiple counties.
• Facilitate Coordinated Entry case conferencing meetings and support referrals and housing placements.
• Serve as primary point of contact for Coordinated Entry questions and system navigation.
• Maintain By-Names-List and knowledge of CoC housing resources.
• Develop training and technical assistance for Coordinated Entry providers and partners.
• Use data systems to monitor activity, referrals, placements, and performance.
• Collaborate with CoC staff to ensure data quality and reporting.
