Job summary

Community Action Pioneer Valley is seeking a Coordinated Entry (CE) Coordinator to lead the Three County CoC’s person-centered, trauma-informed, and equity-focused CE system across Berkshire, Franklin, Hampshire, and the North Quabbin region.

You will oversee daily operations, training, and continuous improvement while collaborating with providers, housing partners, and government agencies to ensure effective access to housing resources.

Qualifications

• Minimum Qualifications/Transferable Skills: 4+ years in human services and office data entry.
• Strong interpersonal communication, both verbal and written.
• Ability to work independently in a fast-paced environment and manage multiple priorities.

Responsibilities

• Manage day-to-day operations of the Coordinated Entry System across Berkshire, Franklin, Hampshire, and North Quabbin region.
• Facilitate Coordinated Entry case conferencing and ensure timely, equitable referrals and housing placements.
• Serve as primary contact for questions, referrals, and system navigation; escalate issues to Program Director as needed.
• Maintain By-Names-List and knowledge of CoC-funded and non-CoC housing resources; coordinate referrals.
• Provide training and technical assistance to providers, assessors, and system users on policies and HMIS requirements.
• Monitor Coordinated Entry activity and data quality using HMIS and other data systems.
• Collaborate with CoC staff and providers to develop performance measures and improve system outcomes.
