Job summary

Sweetwater Care in California is seeking a licensed LVN/LPN to provide nursing care within scope of practice in a long-term care setting. You will follow physician orders, center policies, and applicable guidelines to meet residents' needs.

Responsibilities include charting changes, narcotic counting, and collaboration with physicians during rounds. Prior experience in long-term care and with PointClickCare is preferred; BLS/CPR certification is required or obtainable.

Qualifications

• Experience in long-term care required.
• PointClickCare charting experience desired.
• Active LVN/LPN license in California.
• BLS/CPR certification or willingness to acquire.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin issues, or weight loss.
• Uses the Quality Life Manual for patient placement and monitoring in programs.
• Notifies relevant departments of room changes, LOA, admits, or discharges.
• Participates in rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
