Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within scope of practice in a skilled nursing setting, aligning with physician orders and center policies.

Key duties include accurate charting, incident reporting, narcotic control, and monitoring patient changes. The role requires CA licensure, BLS/CPR, and may involve leadership responsibilities as needed.

Qualifications

• Minimum Long Term Care experience required.
• Experience with Point Click Care charting preferred.
• Current LVN/LPN license in California required; BLS/CPR certification preferred or willing to obtain.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.

• Experience in nursing management is preferred but not required.
