Job summary

Sweetwater Care is seeking an LVN with nursing management experience to lead nursing operations in a California facility. The role requires adherence to infection control policies, participation in quality improvement, and strong coordination with physicians and care teams.

Responsibilities include medication administration, patient and family education, and ensuring timely treatments in line with nurse practice act requirements.

Qualifications

• Currently licensed as LVN in California; compact license acceptable for compact states.
• One year of nursing management experience preferred.
• Experience in long-term care settings is required.
• Proficiency with PointClickCare charting is preferred.

• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders, when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
