Job summary

Sweetwater Care is seeking an experienced LVN to provide nursing care within the scope of practice, under physician orders and facility policies. The role requires coordination with physicians, families and departments to ensure patient needs are met and care plans are implemented.

The candidate should have California LVN licensure, prior nursing management and long-term care experience, and familiarity with PointClickCare charting to document care and communication efficiently.

Qualifications

• Licensed LVN in California is required.
• Prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Long term care experience desired.
• Point Click Care charting experience an advantage.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in and assists with Quality Improvement Program.
• Utilizes Quality Life Manual for patient identification, placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes and implements physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
