Job summary

Sweetwater Care in Alhambra, CA is seeking an LVN with nursing management experience to lead clinical operations within a SNF/LTC setting.

The role requires current California LVN licensure and familiarity with PointClickCare charting. Prior LTC leadership helps ensure compliance, safe patient care, and effective team supervision in a regulated care environment.

Qualifications

• California LVN license required.
• Minimum 1 year nursing management experience preferred.
• SNF/LTC experience preferred.
• Long term care experience.
• PointClickCare charting experience.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, admit, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives/transcribes physician orders and carries them out as allowed by the Nurse Practice Act.
• Might make rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
