Job summary

Sweetwater Care in California is seeking a dedicated LVN/LPN to provide nursing care within scope of practice, following standards and physician orders. This position requires long-term care experience, CA license, and BLS/CPR certification.

You will support charting, incident reporting, and participation in patient care plans alongside healthcare teams. Responsibilities include documenting condition changes, narcotics and pharmacy procedures, and rounds with physicians to ensure optimal patient

Qualifications

• Long-term care experience required.
• Experience with PointClickCare charting.
• Valid LVN/LPN license in CA.
• BLS/CPR certification required or willingness to obtain.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin issues, weight loss, or other changes.
• Uses the Quality Life Manual to identify, place, and monitor patients in programs.
• Documents and notifies relevant departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
