Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care in accordance with standards and physician orders in a long-term care setting.

Qualified candidates hold an active California LVN/LPN license and BLS/CPR certification, with experience in long-term care and charting. Nursing management experience is a plus, though not required. This role emphasizes accurate documentation and collaboration with physicians and departments.

Qualifications

• Minimum long-term care experience.
• Experience with charting and incident reporting is preferred.
• Nurse license in practicing state and current BLS/CPR.
• Nursing management experience is preferred but not required.

Responsibilities

• Follow established procedures for charting and incident reporting.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions (nutrition, infections, skin issues, weight loss, etc.).
• Use Quality Life Manual for patient program placement and monitoring.
• Document and notify departments about room changes, LOA, admits or discharges.
• Make rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
